Why it’s built this way
Nothing to write
Every answer is a choice from a list, a number, or a date. That keeps this a form: it responds to the options you pick, not to a description of your situation.
Nothing to hand over
We don’t ask for names, addresses, amounts, or a start date at all. The template carries a highlighted blank for each one, which you fill in yourself once it’s open in Word.
